血清谷丙转氨酶与代谢综合征相关性被引量:8

目的探讨血清谷丙转氨酶(ALT)与代谢综合征(MS)及其组分之间的关系,为MS的预防控制提供参考依据。方法采样分层整群抽样方法对在江苏省常州市武进区抽取的8818名居民进行问卷调查、体格检查和实验室检测。结果8818名居民中,MS、中心性肥胖、血压升高、空腹血糖(FBG)升高、甘油三酯(TG)升高、高密度脂蛋白胆固醇(HDL—C)降低者分别1907、3264、4674、2369、2843、1591例,分别占21.62%、37.02%、53.00%、26.86%、32.24%、18.04%;ALT平均水平为(22.43±17.71)U/L,不同ALT四分位分组居民比较,随着ALT水平升高,MS、中心性肥胖、血压升高、FBG升高、TG升高、HDL—C降低者比例均呈增高趋势(均P〈0.001);多因素logistic回归分析结果显示,在调整了性别、年龄、文化程度、吸烟、饮酒等因素后,随着ALT水平的升高,Q。组居民发生MS的风险为Q。组居民的5.69倍(OR=5.69,95%CI=4.72~6.86);进一步调整中心性肥胖、血压升高、FBG升高、TG升高、HDL—C降低5个MS组分后,Q。组居民发生中心性肥胖、血压升高、FBG升高、TG升高的风险分别为Q,组居民的3.20倍(OR=3.20,95%CI=2.74—3.75)、1.36倍(OR=1.36,95%CI=1.17~1.58)、1.87倍(OR=1.87,95%CI=1.58—2.20)、3.52倍(OR=3.52,95%CI=2.98—4.15)。结论血清ALT水平与MS及其组分相关,血清ALl7水平可能成为MS患病的一个预测指标。

Cancer risk and key components of metabolic syndrome:a population-based prospective cohort study in Chinese被引量:1

Background The key components of metabolic syndrome (MS) are waist circumference,blood pressure,fast blood glucose,high density lipoprotein cholesterol (HDL-c) and triglycerides (TG).These components have,separately and jointly,been associated with an increased risk of cardiovascular diseases.In this study,we aimed to explore the association between MS components and cancer risk in a population-based cohort in China.Methods We established a population-based cohort with 17 779 individuals aged 35 and above at baseline in 2004 and 2005 in Changzhou,Jiangsu Province,China.All participants were face-to-face interviewed to complete a questionnaire and were accepted physical examinations including blood tests for glucose and lipids and physical measurements for obesity and blood pressure.In 2009,a total of 16 284 subjects (6886 men and 9398 women,91.6%) attended the flow-up interviews and the participants or their family members reported all the hospitalizations and diseases including cancer occurred during the follow-up period.Multivariate Cox regression was used to estimate the hazard ratios (HRs) of metabolic syndrome components and cancer incidence.Results There was a dose-response association between cancer risk and the number of MS components presented at baseline (P for trend=0.012) and the HR (95% confidence interval (CI)) was 2.63 (1.27-5.45) for subjects carrying 3 or more metabolic syndrome components after adjustment for possible confounding factors.Specifically,the multivariate-adjusted HRs (95% CIs) for cancer risk in subjects with central obesity,high fasting glucose,low HDL-c were 1.94 (1.01-3.74),2.04 (1.10-3.77) and 2.05 (1.09-3.88),respectively.Conclusions In this population-based,prospective cohort study in China,we found MS components,e.g.,central obesity,high fasting glucose,low HDL-c were risk factors for cancer development.Early intervention of MS components may be also beneficial to reduce cancer burden.

Genetic variants at 10q 23.33 are associated with plasma lipid levels in a Chinese population被引量:1

Plasma lipid abnormalities are implicated in the pathogenic process of type 2 diabetes. The IDE-KIFII-HHEX gene cluster on chromosome 10q23.33 has been identified as a susceptibility locus for type 2 diabetes. We hy- pothesized that genetic variants at 10q23.33 may be associated with plasma lipid concentrations. Seven tagging single nucleotide polymorphisms (SNPs: rs7923837, rs2488075, rs947591, rs11187146, rs5015480, rs4646957 and rs1111875) at 10q23.33 were genotyped in 3,281 subjects from a Han Chinese population, using the Taq- Man OpenArray and Sequenom MassARRAY platforms. Multiple linear regression analyses showed that SNP rs7923837 in the 3"-flanking region of HHEX was significantly associated with triglyceride levels (P = 0.019, 0.031 mmol/L average decrease per minor G allele) and that rs2488075 and rs947591 in the downstream region of HHEX were significantly associated with total cholesterol levels (P = 0.041, 0.058 mmol/L average decrease per minor C allele and P = 0.018, 0.063 mmol/L average decrease per minor A allele, respectively). However, the other four SNPs (rs11187146, rs5015480, rs4646957 and rs1111875) were not significantly associated with any plasma lipid concentrations in this Chinese population. Our data suggest that genetic variants in the IDE-KIF11- HHEX gene cluster at 10q23.33 may partially explain the variation of plasma lipid levels in the Hart Chinese pop- ulation. Further studies are required to confirm these findings in other populations.

常州市武进区老年人高脂血症流行病学调查被引量:16

目的了解常州市武进区60岁及以上老年人高脂血症及其流行病学分布特征。方法利用整群随机抽样的方法,抽取常州市武进区60岁及以上老年人群4 967人,对所有样本人群进行流行病学调查及相关生化指标检查。结果常州市武进区60岁及以上人群高脂血症患病率为25.91%,标化率为25.93%,女性患病率(29.56%)高于男性(20.84%)(P<0.001),且高脂血症的患病率随年龄增高而下降(P=0.004);其中单纯性高胆固醇血症、单纯性高甘油三酯血症、单纯性低高密度脂蛋白胆固醇血症和混合型高脂血症的患病率分别为4.59%、12.25%、7.13%和1.94%。多因素Logistic分析显示,女性、低年龄组、有高血压史、体重指数(body mass index,BMI)和空腹血糖升高为高脂血症的危险因素。结论常州市武进区60岁及以上老年人群高脂血症患病率较高,应该加强健康教育,开展危险因素的干预,降低慢性病对人群健康的危害。

Fatty acid desaturase 1 polymorphisms are associated with coronary heart disease in a Chinese population

Background A recent genome-wide association study in Caucasians revealed that three loci (rs174547 in fatty acid desaturase 1 (FADS1), rs2338104 near mevalonate kinase/methylmalonic aciduria, cobalamin deficiency, cblB type (MVK/MMAB) and rs10468017 near hepatic lipase (LIPC)) influence the plasma concentrations of high-density lipoprotein-cholesterol (HDL-C) and triglycerides (TG). However, there are few reports on the associations between these polymorphisms and plasma lipid concentrations in Chinese individuals. This study aimed to evaluate the associations between these three polymorphisms with HDL-C and TG concentrations, as well as coronary heart disease (CHD) susceptibility in Chinese individuals.
