The mucosal immune system in the oral cavity——an orchestra of T cell diversity被引量:10

The mucosal immune system defends against a vast array of pathogens, yet it exhibits limited responses to commensal microorganisms under healthy conditions. The oral-pharyngeal cavity, the gateway for both the gastrointestinal and respiratory tracts, is composed of complex anatomical structures and is constantly challenged by antigens from air and food. The mucosal immune system of the oral-pharyngeal cavity must prevent pathogen entry while maintaining immune homeostasis, which is achieved via a range of mechanisms that are similar or different to those utilized by the gastrointestinal immune system. In this review, we summarize the features of the mucosal immune system,focusing on T cell subsets and their functions. We also discuss our current understanding of the oral-pharyngeal mucosal immune system.

3D打印导板辅助髁突骨软骨瘤及继发复杂牙颌面畸形的手术治疗被引量:19

目的:研究3D打印手术导板辅助髁突骨软骨瘤及继发牙颌面畸形同期矫治的临床可行性。方法:根据影像学资料,运用Dolphin Imaging 11.7Premium和Mimics软件进行虚拟外科手术设计并制作3D打印手术导板,指导髁突骨软骨瘤及继发牙颌面畸形的手术矫治。通过对比模拟头颅模型与实际术后CT重建模型评估此方法的临床价值。结果:所有患者患侧关节功能、咬合关系以及颜面对称性都得到了良好的恢复。模拟术后模型与术后扫描重建模型对比中,中切牙与第一磨牙的误差均保持在1.4mm以下,最大误差出现在颏部,约2.4mm,显示了新方法的准确性。结论:结果显示虚拟外科手术设计和手术导板有助于髁突骨软骨瘤继发牙颌面畸形的准确诊断、治疗方案设计、准确截骨以及重置骨块。

Expression of p53, p21^(CIP1/WAF1) and eIF4E in the adjacent tissues of oral squamous cell carcinoma:establishing the molecular boundary and a cancer progression model被引量:1

The present study evaluated the expression of key molecules and the status of DNA in both oral squamous cell carcinoma(OSCC) and adjacent tissues to establish a molecular surgical boundary and provide a cancer progression model. Biopsy samples from 50 OSCC patients were divided into T(cancer), P1(0–0.5 cm), P2(0.5–1 cm), P3(1–1.5 cm) and P4(1.5–2 cm) groups based on the distances from the visible boundary of the primary focus. Twenty samples of normal mucosa were used as controls. We used immunohistochemical staining and flow cytometry to evaluate p53, p21CIP1/WAF1, e IF4 E and Ki-67 expression and to determine DNA status, respectively. Sub-mucosal invasion was present in the P1 and P2 groups as determined by haematoxylin and eosin staining.Mutant p53 expression decreased gradually from cancerous to normal mucosae, whereas p21CIP1/WAF1 expression displayed an opposite trend. e IF4 E expression decreased from cancerous to normal mucosae. Ki-67 expression, the heteroploidy ratio, S-phase fraction and proliferative index decreased gradually with the distance from the tumour centre. Based on these results, we suggest that the resection boundary in OSCC surgery should be beyond 2 cm from the tumour. Additionally, the adjacent tissues of the primary focus could be used as a model for assessing cancer progression.

Interferon-c and interleukin-4 detected in serum and saliva from patients with oral lichen planus被引量:10

Our previous salivary study had demonstrated an apparent T helper 2(Th2)-predominance in saliva of oral lichen planus(OLP) patients and suggested a potential of salivary interleukin-4(IL-4) as a biomarker for monitoring disease severity. To further determine the consistency of Th1/Th2 bias of OLP, this study investigated the expression profile of interferon-c(IFN-c) and IL-4 in serum and the relationship of the serum levels of these cytokines with their saliva partners. Sixty ethnic Chinese patients with OLP(40 of the erythematous/ulcerative form and 20 of the reticular form) were recruited for this study, with 40 age–sex-matched healthy volunteers as control group. IFN-c and IL-4 levels in serum and paired saliva samples were screened by enzyme-linked immunosorbent assay. OLP patient showed a low-level IFN-c but high-level IL-4 expression profile in both serum and saliva, with a lower IFN-c/IL-4 ratio. Serum IL-4 level in the erythematous/ulcerative group was significantly higher than that in the reticular group. Serum levels of IFN-c and IL-4 were significantly and positively correlated with their saliva partners. These results provided more evidence for Th2 cytokinepredominant immune imbalance in OLP, as well as the potential of IL-4 as the biomarker for monitoring severity of OLP.

FGF8 induces epithelial-mesenchymal transition and promotes metastasis in oral squamous cell carcinoma被引量:2

Oral squamous cell carcinoma(OSCC)is one of the most common cancers worldwide,and with 354864 new cases each year.Cancer metastasis,recurrence,and drug resistance are the main causes to cripples and deaths of OSCC patients.As potent growth factors,fibroblast growth factors(FGFs)are frequently susceptible to being hijacked by cancer cells.In this study,we show that FGF8 is upregulated in OSCC tissues and high FGF8 expression is related with a set of clinicopathologic parameters,including age,drinking,and survival time.FGF8 treatment enhances the invasive capability of OSCC cells.Lentivirus-based FGF8 expression promotes OSCC metastasis in a mouse lung metastasis model.Further,mechanistic study demonstrates that FGF8 induces epithelial–mesenchymal transition(EMT)in OSCC cells.These results highlight a pro-metastatic function of FGF8,and underscore the role of FGF8 in OSCC development.

The worldwide incidence of bone disorders and conditions has been increasing. Bone is a nanomaterials composed of organic(mainly collagen) and inorganic(mainly nano-hydroxyapatite) components, with a hierarchical structure ranging from nanoscale to macroscale. In consideration of the serious limitation in traditional therapies, nanomaterials provide some new strategy in bone regeneration. Nanostructured scaffolds provide a closer structural support approximation to native bone architecture for the cells and regulate cell proliferation, differentiation, and migration, which results in the formation of functional tissues. In this article,we focused on reviewing the classification and design of nanostructured materials and nanocarrier materials for bone regeneration, their cell interaction properties, and their application in bone tissue engineering and regeneration. Furthermore, some new challenges about the future research on the application of nanomaterials for bone regeneration are described in the conclusion and perspectives part.

高速涡轮机法与增隙经舌侧法在下颌阻生齿拔除术中的效果对比被引量:30

目的对比研究高速涡轮机法与增隙经舌侧法在下颌阻生齿拔除术中的临床效果。方法收集2013年3月至2014年3月我科收治的354例下颌近中阻生齿患者,随机分为高速涡轮机组(176例)与增隙经舌侧组(178例),比较两组患者手术时间、术后张口受限、颞下颌关节区疼痛、局部肿胀等并发症发生情况,以及拔牙患者对两种术式手术操作后的感受情况,是否对拔牙产生恐惧感,并对结果进行统计学分析。结果两组患者的手术时间及术后张口受限、颞下颌关节区疼痛、局中部肿胀在近中中、高位阻生智齿患者中差异均无统计学意义(X^2分别为2.18、0.27、0.67、0.46,P>0.05);在近中低位、水平阻生智齿患者中拔牙时间、张口受限、颞下颌关节区疼痛、局中部肿胀差异有统计学意义(X^2分别为32.88、4.53、5.26、4.55,P<0.05)。增隙经舌侧组有90人产生了恐惧感,高速涡轮机法组有68人产生了恐惧感,差异有统计学意义(X^2=5.09,P<0.05)。结论在近中中、高位阻生智齿拔出术中,两种手术方式都能取得较好效果。在近中低位、水平阻生智齿拔除术中,高速涡轮机组的临床效果要优于传统术式。在临床应用中,相对患者的心理感受来讲,高速涡轮机法更能让患者接受,能够降低患者对拔牙的恐惧感。

地图舌危险因素的研究进展被引量:6

地图舌(良性游走性舌炎)是一种常见的浅表性良性舌部炎症,临床表现为丝状乳头剥脱红斑区伴周围黄白条带状微凸边缘,病理为非特异性炎症表现,大多数患者无症状,病损可自愈,常有复发,一般不需药物治疗。当病损累及舌部以外口腔黏膜时称异位性地图舌(游走性口炎),其临床表现及组织病理学特点均似地图舌。地图舌病因尚不明确,与年龄相关,儿童多发;危险因素复杂,包括免疫因素、遗传因素、过敏体质、精神压力、吸烟、内分泌因素以及缺锌等;可伴发其他疾病如沟纹舌、银屑病、糖尿病、胃肠紊乱、灼口综合征或唐氏综合征等。地图舌是否为银屑病的口腔表征尚存争议,其与沟纹舌关系尚待进一步研究。本文就地图舌危险因素的研究进展作一综述。

手术机器人辅助手术自21世纪以来逐渐应用于颅颌面外科手术中,除肿瘤性疾病外,在非肿瘤性疾病中的应用也十分广泛,本文就手术机器人在颅颌面非肿瘤性疾病治疗中应用的研究进展作一综述。
